Australian Institute for Human Longevity Pty Ltd ("AIHL", "we", "our", "us") provides health screening services and facilitates participation in clinical research. AIHL is not a medical practice, hospital, or registered health service. AIHL does not provide medical advice, diagnosis, or treatment.

No doctor-patient relationship is created by virtue of membership or use of AIHL services. Members should continue to consult their own general practitioner or specialist for medical advice and treatment.

Eligibility. Membership is open to adults aged 18 and over who are residents of Australia. AIHL reserves the right to decline or terminate membership at its discretion.

Application. By submitting a membership application, you represent that the information you have provided is accurate and complete.

Founding member pricing. Founding member rates are locked for 3 years from the date of joining, provided membership remains continuous. Any break in membership will result in loss of the founding member rate.

Membership fees are billed annually in advance or monthly by direct debit. There is no deposit or joining fee. On monthly plans, the first payment for Tier 2 and Tier 3 is higher, reflecting the onboarding work, testing, pathology and imaging arranged at the start of membership. Fees are set out on the AIHL website at the time of enrolment.

AIHL is not a Medicare provider. Membership fees are not claimable under Medicare. Some private health funds may provide a benefit for certain services — members are responsible for confirming this with their insurer.

Fees are subject to change. Members will receive 60 days' notice of any fee increase. Founding members are not subject to fee increases for the duration of continuous membership.

Members may cancel their membership by providing 30 days' written notice to info@aihl.au.

Cancellations take effect at the end of the 30-day notice period. Membership benefits remain active during this period. On cancellation, any refund is calculated on a pro-rata basis, taking into account the testing, services and work already provided. Services already provided (for example, completed pathology, imaging or reviews) are not refundable. The first payment is refundable only if cancellation is requested before any services have been provided.

AIHL may terminate membership immediately if a member breaches these Terms or acts in a manner inconsistent with the values of the programme.

As a condition of AIHL membership, all members agree to contribute de-identified health data to the AIHL Research Registry. This data is used for longevity research conducted by AIHL and in partnership with Griffith University and other approved research collaborators.

De-identified data cannot be used to identify any individual member. Participation in clinical trials beyond the Research Registry is entirely voluntary and requires separate informed consent.

You acknowledge that your de-identified Research Registry contribution will continue to be used in completed research even after you cancel your membership.

AIHL provides:

— Health screening and longitudinal biomarker testing

— Interpretation of biomarker results by AIHL clinical staff

— Access to the AIHL member dashboard

— Facilitation of voluntary participation in approved clinical trials

— Access to the AIHL member community

AIHL does not provide:

— Diagnosis of any medical condition

— Medical treatment or prescriptions

— Emergency medical services

If you are experiencing a medical emergency, call 000 immediately.

To the maximum extent permitted by law, AIHL's liability to any member is limited to the membership fees paid in the 12 months preceding the event giving rise to the claim.

AIHL is not liable for any indirect, consequential, or economic loss arising from membership or use of AIHL services.

Nothing in these Terms limits any rights you may have under the Australian Consumer Law.

By becoming a member, you consent to:

— AIHL collecting, storing, and using your health information as described in our Privacy Policy

— AIHL sharing relevant health information with its clinical team members

— Your biomarker results being coordinated between AIHL and approved pathology and imaging providers

— Your de-identified data being contributed to the AIHL Research Registry

You may withdraw consent for specific uses by contacting us at info@aihl.au, subject to any obligations that have already been fulfilled.

Members must not:

— Use AIHL services for any unlawful purpose

— Attempt to access data belonging to other members

— Misrepresent their identity or health information to AIHL

— Use AIHL content, research findings, or materials for commercial purposes without written consent

All content on the AIHL website and member platform — including research findings, reports, educational materials, and branding — is the intellectual property of AIHL or its licensors. You may not reproduce or distribute this content without prior written permission.

These Terms are governed by the laws of Queensland, Australia. Any dispute arising from these Terms will be subject to the exclusive jurisdiction of the courts of Queensland.

We may update these Terms from time to time. We will notify members of material changes by email at least 30 days before they take effect. Continued membership after the effective date constitutes acceptance of the updated Terms.
