Australian Institute for Human Longevity Pty Ltd (ACN to be confirmed) ("AIHL", "we", "our", "us") operates the AIHL membership programme and clinical research platform. Our registered address and contact details are available at aihl.au/contact.

AIHL is not a medical practice and does not provide medical advice, diagnosis, or treatment. We operate as a health screening and clinical research facilitation service.

We collect the following categories of personal information:

Identity information: Full name, date of birth, sex, contact details (email address, phone number, postal address).

Health information: Biomarker test results, body composition data, medical history provided for clinical screening purposes, and any information you share with our clinical team.

Payment information: We collect sufficient information to process membership fees. Full payment card details are held by our payment processor, not by AIHL.

Research Registry data: De-identified health data contributed to the AIHL Research Registry as a condition of membership. This data cannot be used to identify you.

Usage data: Information about how you use the AIHL member dashboard, including login times and feature usage, collected for service improvement purposes.

We collect personal information:

— Directly from you when you apply for membership, complete health questionnaires, or contact us

— From pathology providers who conduct your biomarker tests on AIHL's instruction

— From imaging providers who conduct your body composition scans

— Automatically through your use of the AIHL member platform

We use your personal information to:

— Administer your AIHL membership and deliver programme services

— Coordinate and communicate your test results

— Facilitate your participation in clinical trials (if you consent separately)

— Contribute de-identified data to the AIHL Research Registry

— Process membership fees and manage your account

— Communicate programme updates, research findings, and service changes

— Comply with legal and regulatory obligations

We do not use your identifiable health information for marketing purposes. We do not sell your personal information to third parties.

As a condition of AIHL membership, all members contribute de-identified health data to the AIHL Research Registry. This data is used for internal and collaborative research into longevity and ageing biology.

De-identified data cannot be used to identify you. It does not contain your name, contact details, or any other directly identifying information. The de-identification process is conducted in accordance with the Australian Privacy Act 1988 and relevant health privacy guidelines.

Participation in clinical trials (beyond the Research Registry contribution) is entirely voluntary and requires separate informed consent.

We may disclose your personal information to:

— Pathology and imaging providers to coordinate your testing

— Our clinical team members who provide oversight of your programme

— Griffith University, our academic research partner, for research purposes under formal agreement (de-identified data only, unless you separately consent otherwise)

— Payment processors for the purpose of fee collection

— IT service providers who support our platform infrastructure

— Regulatory bodies where required by law

We do not disclose your identifiable health information to any third party for commercial or marketing purposes.

Some of our service providers and platform infrastructure may process data outside Australia. Where this occurs, we take reasonable steps to ensure that overseas recipients handle your information in accordance with the Australian Privacy Principles.

Your personal information is stored on secure cloud infrastructure with access controls, encryption at rest and in transit, and regular security reviews. Health information is treated with the highest level of security appropriate to its sensitivity.

We retain your personal information for as long as your membership is active and for a reasonable period thereafter to comply with legal obligations and resolve any disputes.

You have the right to:

— Access the personal information we hold about you

— Request correction of inaccurate or incomplete information

— Make a complaint about our handling of your information
